Who We Are

New Horizon Counseling Center (NHCC) is a leading non-profit behavioral health organization licensed by the New York State Office of Mental Health (OMH), and New York State Office for People with Developmental Disabilities (OPWDD) and New York State Office of Alcoholism and Substance Abuse Services (OASAS). Our excellent reputation reflects over 30 years of experience in meeting the needs of the communities we serve. We service communities in Brooklyn, Queens, Nassau and Suffolk Counties.

What We Do

The Care Coordination Program provides services as designed by the NYS DOH Health Home Model, based on unified systems of care to coordinate and integrate physical and behavioral health care, substance use treatment, and social services. The goal of Care Coordination is to minimize the use of hospitals and emergency rooms by providing access to all of the services an individual needs to assure he or she stays healthy.
